WHAT IS MENOPAUSE?

Perimenopause refers to the years immediately preceding menopause and is characterized by hormonal changes which often lead to menopause symptoms and can occur between 35 and 60. It usually lasts for a period of 2 to 6 years. Perimenopause is technically defined as the time from which menses start to become irregular through until 12 months after the last menstrual bleed.

Menopause is the permanent termination of a woman’s period and her fertility and is confirmed when a woman doesn’t have a period for twelve consecutive months. The menopausal climax years include the year or two before and a year or more after your very last menstruation. The average age of a woman in the midst of her Change is 51. But women come to their menopausal climax in their 20s, 30s, and 40s, as well. Some achieve menopause by surgical means, some by way of chemotherapy or radiation, and some just naturally arrive early. Menopausal climax before the age of 40 is considered "premature."

Post menopause refers to the stage after a woman’s last period. Due to a decline in hormone levels, this stage brings with it a new set of health concerns, including heart disease and osteoporosis. The term is applied to women who have not experienced a menstrual bleed for a minimum of 12 months, assuming that they do still have a uterus, and are not pregnant.

Physical symptoms of menopause can include:

  • Hot flushes, including night sweats
  • Atherosclerosis
  • Migraine
  • Rapid heartbeat
  • Thinning of the membranes of the vulva, vagina, cervix and outer urinary tract. Vaginal itching. Watery discharge. Decreased libido. Uncomfortable intercourse due to the thinning of the vaginal walls causing dryness and loss of elasticity.
  • Irregular, scant or heavy periods. Irregular or heavy bleeding can be caused by conditions other than the menopause e.g. fibroids or polyps, so it's a good idea to get it checked out.
  • Osteoporosis
  • Breast tenderness
  • Formication (tingling, burning, pins and needles, or sensation of ants crawling on or under the skin)
  • Dry or itchy skin due to thinner skin (without oestrogen our skin finds it difficult to retain moisture).
  • Aches and pains caused by low levels of oestrogen.
  • Insomnia caused by low levels of oestrogen (exacerbated by night sweats) or brought on by anxiety or depression.
  • Tiredness and lethargy due to night sweats and insomnia.
  • Bladder problems e.g. cystitis or other bladder infections and stress incontinence. Stress incontinence is caused by loss of muscle and tissue elasticity in the pelvic cavity, which leads to a tendency to leak urine on coughing or laughing. Some women also experience reduced bladder capacity.

Anxiety, depression, sadness, difficulty concentrating, overreacting to minor upsets, anger and irritability, forgetfulness and mood swings are all typical psychological problems. Hormonal changes are thought to be responsible for a proportion of these symptoms, but it is hard to be certain which symptoms are hormonal and which are due to other changes in a woman’s life around that time. Studies indicate that many cases of depression, for example, relate more to personal circumstances than to the menopause itself. Depression may be more likely in the years immediately before the menopause, especially if you've experienced PMS in the past. Studies have also indicated that women who are generally happy with their lives experience fewer problems during menopause. Situations and life events that tend to crowd into women’s lives when in their late forties and fifties include: children leaving home, divorce, retirement (of self or partner), widowhood, illness or death of parents and physical ageing. Other psychological challenges can include beliefs about no longer being useful, distorted body image, fear of death, feeling ‘unemployable’ and low self worth.
